DR. SHANTEL CROSBY, LCSW Greetings!! I'm Dr. Crosby--owner of West Rose Therapy & Consultation, LLC, a clinical therapist, social work professor, and consultant. I earned a BA in Psychology, an MSW, and a PhD in Social Work, focusing on trauma and trauma-informed practice. I'm currently licensed to work with residents of Kentucky & Michigan and provide all services through web-based video conferencing. While I operate in various roles, the thread that runs through them all is my passion for helping and empowering others. As a clinician, my goal is to engage clients in a restorative relationship, using an ecological lens and client-centered approaches. I work with adult clients dealing with depression or anxiety, seeking to heal from past or present trauma, or simply needing support in managing difficult life transitions. Given the unique nature of each client's experience, I utilize an eclectic blend of evidence-informed modalities and techniques to help you achieve your goals. Outside of work, my faith, family, and friends are the most important things to me! I'm a Detroit, MI native, but currently reside in Louisville, KY. I'm also a foodie who enjoys a good book or podcast, as well as traveling! Choosing a therapist is a big decision and finding the right fit is imperative. THERAPY FEES & INSURANCE As an out-of-network provider, fees for services (listed below) are billed directly to clients as self-pay and clients are provided with a Good Faith Estimate prior to their first session. Clients may also be able to seek reimbursement from their insurance company for service fees that they have paid--West Rose can provide you with a monthly superbill to submit to your insurance company for this reimbursement. However, all clients seeking this option should contact their insurance company in advance to verify out-of-network benefits provided in their coverage and to confirm eligibility. GOOD FAITH ESTIMATES You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. This means that: (1) You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees; (2) You should make sure that you receive a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask for a Good Faith Estimate before you schedule an item or service; (3) If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill; (4) You should make sure to save a copy or picture of your Good Faith Estimate. ORGANIZATIONAL CONSULTATION SERVICES West Rose also provides consultation services to organizations (e.g., schools, community agencies, etc.) to address trauma/trauma-informed practice, equity, and organizational culture.
